I am EXTREMELY afraid of inadvertently eating bugs. Consequently, I constantly have to have a view of my plate or bowl, and eat quickly so that no bugs get in. If I suspect a bug MAY have gotten into something in a cupboard, I will throw the whole container away. It's getting to be unmanageable...it costs a lot of money to replace the food, I don't eat as much, and I have kids so I don't want them to get affected by it. I just recently realized I have this fear, though I'm 32 years old. There's no specific name for this one, but I would name it "entomophagophobia" which is a similar name to the fear of bugs generally--but with the addition of the eating ("phago") of bugs.
